Medical Records

A complete record of your care and treatment is kept in our medical records department. These records are strictly confidential, and only authorized persons have access to them. A copy of your medical record will be provided upon written request and for a reasonable fee. To request a copy of your medical record, please send a completed authorization form with a copy of your driver’s license by mail/fax back to the address or fax number listed below. Please include your telephone number on the authorization form so we can contact you.

Although your medical record is the physical property of the facility, you have the right to inspect or obtain a copy of the health information. For more information you may visit the Medical Records Department between Monday – Friday, 8a-5:00p or call Release of Information at 409-938-5176.
